Preventive care covered at 100% (deductible waived), basic services covered at 60% and major services covered at 50%. Maximum Rewards Program allows you to increase your annual maximum benefit over time if you go to the dentist at least once each calendar year and use less than $250 of your annual benefit per year. PPO Network available; however, you may use any dentist without penalty. |
